A: They're located under "%appdata%\.minecraft\saves". Each folder labled World(X) contains the files that are used for the world data.
A: You need to make an account (free) and then verify the account. After that, the upload link will appear on the navigation bar and you will be able to upload your maps. The site takes in a zip compressed file of the world data. Zip the entire folder {"World(1,2,3...)} so all the required files are there for people to use your game files.
A: Unzip the world files to your local save folder. Then load the game and select the world that you wish to play.
A: You broke something. Way to go. It generates an email when that happens so I can see what is broken. It'll be patched in a future release.
A: First off, they need to be zip files under 20MB. Secondly, you must be able to upload the file in under 20 minutes. Lastly, I set a limit to 5 maps per user account for the beta period. That limit can be changed, just ask me.
A: Only users that log in can vote and you only get to vote once per map per day.
